Trombose de veia central da retina em paciente usuária de interferon e ribavirina: relato de caso / Central vein occlusion in a patient using interferon and ribavirin: case report

O interferon alfa (INF alfa) é droga atualmente utilizada no tratamento de várias doenças sistêmicas, como a hepatite C crônica. A ribavirina quando associada ao interferon alfa aumenta muito a resposta ao tratamento. Estima-se que a infecção crônica pelo vírus da hepatite C afete 170 milhões de pessoas no mundo, muitas delas em uso dessas medicações. A forma típica da retinopatia associada ao interferon alfa apresenta exsudatos algodonosos e hemorragias intra-retinianas. Há vários relatos de alterações oculares associadas ao uso do interferon alfa. Este trabalho descreve um caso de oclusão de veia central da retina em olho direito, com hemorragias no olho contralateral, em paciente usuária dessas medicações por dois anos. O caso descrito expõe em um dos olhos o quadro mais freqüente da retinopatia associada ao uso de interferon alfa (hemorragias de fundo) e no olho contralateral, uma apresentação muito mais atípica (trombose de veia central da retina). O quadro fundoscópico apresentou melhora com a interrupção da medicação.

Interferon and ribavirin are medications widely used in the treatment of some systemic diseases, mainly hepatitis C. Ribavirin when associated with interferon increases the rate of success of this treatment. There are about 170 million patients with chronic hepatitis C in the world, many in use of these medications. The classic associated retinopathy is described as cotton wool exudates and hemorrhages. Since the first reports, several different ocular disturbances were described in association with interferon. The present case shows a patient whose right eye presented with central retinal vein occlusion and whose left eye presented the typical findings of hemorrhages; prompt resolution after the medications were discontinued.

Retinopatia causada pelo uso de interferon em pacientes portadores de hepatite C / Retinopathy caused for the use of interferon in carrying patients of hepatitis C

Objetivo do estudo é relatar casos de retinopatia causada pelo uso de interferon e discutir suas características clínicas e evolução. O método empregado foi um estudo descritivo, com relato de casos de pacientes portadores de hepatite C, que apresentaram retinopatia pelo uso do interferon. O uso de interferon pode causar retinopatia nos pacientes em tratamento, o tempo de aparecimento das lesões é variável (4 a 7 semanas). As lesões retinianas, hemorrágicas e exsudativas, geralmente desaparecem durante terapia ou logo após sua interrupção. A maioria dos pacientes que desenvolve a retinopatia pode continuar a terapia com interferon sem maiores prejuízos à visão. É necessário, para os portadores de hepatite C, uma avaliação oftalmológica antes do início da terapia e avaliações periódicas, especialmente se for constatada baixa de acuidade visual (AU)

Objective of the study is to tell cases of retinopathy caused for the use of interferon and to argue its clinical characteristics and evolution. The employed method was a descriptive study, with story of cases of carrying patients of hepatitis C, that presented retinopathy for the use of interferon. The use of interferon can cause retinopathy in the patients in treatment, the time of appearance of the injuries is changeable (4 to 7 weeks). The retinal, hemorrhagic and exudatives injuries, generally disappear during therapy or soon after its interruption. The majority of the patients that develops the retinopathy can continue the therapy with interferon without bigger damages to the vision. It´s indisplensable, for the carriers of hepatitis C, a ophthalmologic evaluation before the beginning of the therapy and periodic evaluations, especially if have been evidenced low of visual acuity (AU)

Metakelfin toxicity--with pale centered superficial retinal haemorrhages.

An interesting case of acute onset bilateral superficial white-centered retinal haemorrhages probably due to idiosyncrasy to a new antimalarial drug is reported.
